Kim Si-a made her acting debut in 2018 with the short film Wish Stone Tower and the feature Miss Baek, in which she played Kim Ji-eun, a young girl facing abuse. She secured the role through a highly competitive 600-to-1 audition process. Her performance earned critical praise and multiple awards, including Best Child Actor at the Golden Cinematography Awards and Best Actress at the Sharm El-Sheikh Asian Film Festival.
She followed this with roles in The House of Us (2019) and the blockbuster disaster film Ashfall (2019). In 2020 she starred in the horror film The Closet. On the streaming front, she gained international visibility as young Ashin in Kingdom: Ashin of the North (2021) and appeared in The Silent Sea (2021). She later played Gil Jae-yeong in the Netflix action film Kill Boksoon (2023) and Seo Yi-su in Sweet Home Seasons 2 and 3 (2023–2024).
